PointCare to Unveil Enrollment Tracker at HIMSS Annual Conference
San Mateo, Calif. – January 29, 2014 – For thousands of community health clinics in need of more easily confirming eligibility and enrolling millions of newly-eligible patients in health plans, PointCare, an enrollment and reporting software company with roots in the non-profit world, will be demoing Enrollment Tracker at Exhibit Hall booth #6484 during HIMSS14, February 23-27 in Orlando, FL.
Enrollment Tracker is an enhanced feature of PointCarePA, a web-based, mobile-friendly health coverage screening, enrollment, and reporting tool. PointCarePA has assisted its clients with screening more than 560,000 Americans. With proven eligibility and enrollment programs, it offers clinics results-oriented patient tracking and enrollment functionality with the real-time reporting that Federally Qualified Health Centers need to meet ACA accountability requirements.
How It Works
PointCarePA begins its job at the clinic in-take desk or during community outreach with the patient being asked a 90-second, 5-step screening assessment. This assessment scans a robust database of several thousand U.S. health coverage programs, including state and federal health exchanges, and allows clinic staff members to instantly present a unique list of coverage options to each patient. This list includes contact information, costs, program applications, and sign-up checklists of verification documents. Once presented, clinic staff collect contact information and generate a personalized Eligibility Profile.
From there, the Enrollment Tracker auto-populates with the Eligibility Profile information and enables any navigator, certified enrollment counselor, or other clinic staff member to initiate the enrollment of the patient into the coverage option of his or her choice. For each coverage option presented to the patient, the Enrollment Tracker presents the sign-up checklist of enrollment documents and contains a user-friendly, centralized platform for uploading and maintaining all of these documents.
It also gives staff members the ability to track patient enrollment statuses, take notes on patient activities and needs, and maintain clear communication of next steps among multiple users.
While the staff members are using the Screening Assessment and Enrollment Tracker features of PointCarePA, the Dashboard Reporting feature is aggregating all collected demographic data, along with performance information on staff assists and enrollments, which the clinic managers can view in real-time.
