Greenway Health Solutions NCQA PCMH-Prevalidated to Foster Nationwide Care Coordination, Improved Outcomes

Greenway Healthcontinues to lead efforts to improve nationwide population health and care coordination by delivering solutions that support value-based programs such as the National Committee for Quality Assurance (NCQA) patient-centered medical home. PCMH is a primary care delivery model designed to improve health outcomes and reduce healthcare costs while enhancing patients’ and providers’ care experience.

Greenway’s SuccessEHSelectronic healthcare record (EHR) and practice management (PM) solution is now prevalidated by NCQA to receive 27 points in auto credit toward PCMH 2011 scoring — one of the highest information technology solution scores published to date.

All three Greenway® EHR/PM solutions have now achieved prevalidation status for NCQA PCMH, including PrimeSUITE® and Intergy. Using any of these Greenway solutions, practices can streamline the reporting process associated with participating in this important value-driven initiative.

NCQA PCMH is a method to organize primary care practices into medical homes that emphasize care coordination and communication to transform primary care into what NCQA calls “what patients want it to be.” Medical homes can lead to both higher quality and satisfaction as well as lower costs, and NCQA states that its PCMH program is the most widely used way to transform these practices into medical homes.

“PCMH contributes to the goals of improving health outcomes and population health, which closely aligns with Greenway’s goal to help practices deliver high-quality care efficiently to improve health,” said Greenway Health CEO Tee Green. “We’re proud of the accomplishments of so many Greenway customers that have attained PCMH recognition, and to offer three solutions that make it easier for providers to achieve this recognition and work toward these important health-improvement goals.”

Helping providers and consumers better manage health

The PCMH program and medical homes encourage healthcare consumers to more fully engage in managing their own health, while helping providers leverage the most effective tools to deliver care and coordinate with other organizations such as specialist practices, hospitals, labs and pharmacies to prevent errors and unnecessary duplication of care.

PCMH reporting can also facilitate a practice’s participation in other quality reporting incentive programs such as meaningful use and the Physician Quality Reporting System (PQRS), which have like-minded goals for preventive care, chronic disease management and patient compliance.

‘Changing the way we deliver care’

“By providing software that helps physician practices build the patient-focused, coordinated care infrastructure expected of NCQA patient-centered medical homes, Greenway is helping practices be part of modern healthcare’s most important transformations,” said NCQA President Margaret E. O’Kane. “Medical homes are the way of the future, and Greenway is helping practices get there.”

“Our goal is population health…to treat the high-risk patient holistically through quality care,” said Lara Hudson, clinical programs manager at Nanticoke Physician Network in Seaford, DE, which uses Greenway SuccessEHS and was recently awarded the highest PCMH recognition, Level 3 — the fourth organization to do so in Delaware.

“It’s changed the way we deliver primary care,” Hudson stated. “We’re already seeing the benefits now, even financially. Quality is the new driver in healthcare, and PCMH is the new model in care delivery.”
