FRISCO, Texas–(BUSINESS WIRE)–ZeOmega Inc., the premier provider of software solutions for population healthcare management, today announced that BJC HealthCare has selected ZeOmega’s Jiva population healthcare management software to enhance coordination and improve patient engagement for more than 20,000 patients in the BJC Accountable Care Organization (ACO). Based in St. Louis, Mo., BJC HealthCare is one of the largest nonprofit health care organizations in the country.
ZeOmega’s Disease Management (DM) solution components, including care analytics, care plans, and configurable assessments, will be integrated into the population health initiative for the BJC Accountable Care Organization, as well as BJC’s self-insured employee membership, and throughout the organization.
With growing population health management needs, including its employees and dependents, BJC was seeking a solution that would enhance management of patient care transitions. Jiva also will provide the ability to help identify BJC ACO patients requiring intervention by care managers or other clinicians in order to develop highly personalized care plans.
In addition to its care coordination capabilities, Jiva will enable BJC to integrate disparate data into one central location, and enable customization and distribution of correspondence based on user specification and preference for the post discharge process.
ZeOmega says its Jiva solution helps its clients to manage both the clinical and financial risk associated with today’s health care. “Providers and their care teams are responsible for 81 percent of where each healthcare dollar is spent today,” says Sam Rangaswamy, founder and CEO of ZeOmega. “Our Jiva solution enables ACO programs to clearly identify, measure and manage risk, both from a financial and clinical perspective, while supporting better care team and patient engagement.” Rangaswamy adds, “We are honored to support the continued growth and innovation of BJC HealthCare for their accountable care management strategies, and look forward to the impact our solution will have on patient care for health systems and ACOs like BJC.”
ZeOmega will demonstrate Jiva and its capabilities to help ACOs address accountable care requirements at this year’s HIMSS14 Conference at booth #4373.
