SuccessEHS Client Receives Level III PCMH Recognition

BIRMINGHAM, Ala., Oct. 1, 2013 /PRNewswire/ — SuccessEHS client First Care Clinic in Hays, Kan., recently received Level III PCMH recognition from The National Committee of Quality Assurance (NCQA). First Care Clinic was awarded recognition for using evidence-based, patient-centered practices that focus on coordinated care and integrating patients as active participants in their care. This highest level of recognition serves as confirmation of the clinic’s efforts to provide high-quality, affordable care.

The PCMH care delivery model is designed to strengthen patient-physician relationships by moving from reactive care to proactive care, allowing a physician-led care team to facilitate an ongoing relationship for wellness and long-term healing. SuccessEHS provides EHR software and a comprehensive PCMH toolkit that serves a guide for clients as they transition to the PCMH model.

These certified product-versions may require one-time costs to establish, as well as ongoing monthly costs to maintain, interfaces for: reporting to immunization registries, public health agencies, cancer registries and for reporting specific cases to specialized registries. One-time and ongoing monthly costs may also be required for interfaces with LabCorp or Quest or for custom interfaces.
