First Care Clinic was awarded recognition for using evidence-based, patient-centered practices that focus on coordinated care and integrating patients as active participants in their care. This highest level of recognition serves as confirmation of the clinic’s efforts to provide high-quality, affordable care.

The PCMH care delivery model is designed to strengthen patient-physician relationships by moving from reactive care to proactive care, allowing a physician-led care team to facilitate an ongoing relationship for wellness and long-term healing. These certified product-versions may require one-time costs to establish, as well as ongoing monthly costs to maintain, interfaces for: reporting to immunization registries, public health agencies, cancer registries and for reporting specific cases to specialized registries.
