South Nassau Communities Hospital chooses Allscripts dbMotion and FollowMyHealth

CHICAGO, Jan. 24, 2014 /PRNewswire/ — South Nassau Communities Hospital, leveraging and building upon its Allscripts (NASDAQ: MDRX) Sunrise™ electronic health record platform, has selected Allscripts dbMotion and FollowMyHealth to provide system integration, data exchange, patient engagement and next-generation population health management and analytics to coordinate and improve patient care.

In addition, South Nassau has also announced that it will become a Reference Site as part of Allscripts Reference Site Program. An Allscripts Reference Site partner is a health organization, system or office that utilizes an Allscripts solution or set of solutions in a best practices environment. An Allscripts Reference Site allows healthcare IT professionals and decision-makers to see how the Allscripts Open and integrated platform can connect and help optimize patient care.

South Nassau, located in Oceanside, N.Y., has been providing high quality health care to its surrounding communities since 1928. The hospital will employ dbMotion across multiple outpatient facilities and with over 800 providers across the Acute, Ambulatory and Post-Acute environments. South Nassau recently received Magnet status from the American Nurses Credentialing Center (ANCC), recognizing the organization for quality patient care, nursing excellence and innovation in their nursing practice.

“We chose dbMotion because of its integration, data management and population health management capabilities,” said John Mertz, Chief Information Officer, Vice President Information Services for South Nassau. “It is a comprehensive solution, fits seamlessly with our Allscripts Sunrise platform and delivers insights through real-time analytics for more effective care, one patient at a time, through care transitions and irrespective of setting.”

“We are also delighted that South Nassau is now an Allscripts Reference Site,” Mr. Mertz added. “We have many Allscripts solutions employed at our hospital and believe we are an excellent example of how greatly a highly functioning, integrated and innovative system can facilitate the delivery of quality patient care.”

As an Allscripts Reference Site, South Nassau employs Allscripts Sunrise as the foundation of major Allscripts solutions such as Allscripts Sunrise Emergency Care, Sunrise Pharmacy™, Knowledge Based Medication Administration, Enterprise EHR™, and Homecare.  South Nassau is also a client of Allscripts remote hosting and outsourcing services. South Nassau has expanded its relationship with Allscripts in the Population Health space with FollowMyHealth patient engagement solution in addition to its selection of dbMotion.

South Nassau will use dbMotion to harness aggregated and harmonized data to improve care, care transitions, and efficiencies across its organization. The hospital will also implement the Allscripts Population Health Analytics solution, which provides analytics for high-cost, high-priority chronic diseases such as diabetes, heart disease, coronary artery disease and asthma. The data will enable physicians to identify target populations, measure performance and outcomes, analyze utilizations, manage risk and access these insights within their workflow.

“Value-based care requires optimal care coordination and transitions of care inside and outside the hospital or health system,” said Peter McClennen, General Manager, Population Health, for Allscripts. “The coordination and analytical insights dbMotion will generate will help our Allscripts Reference Site partner South Nassau provide even more effective and efficient care to patients across the continuum.”

In addition to helping manage care transitions, dbMotion 5.0 (Modular), an EHR Module, is compliant with the Health Information Technology Standards, Implementation Specifications and Certification Criteria for Electronic Health Record Technology, 2014 Edition in accordance with certain transitions of care certification criteria adopted by the Secretary of Health and Human Services supporting the Stage 2 meaningful use measures required to qualify eligible providers for funding under the HITECH Act.

dbMotion has been certified by an ONC-ACB in accordance with the applicable certification criteria adopted by the Secretary of the U.S. Department of Health and Human Services. This certification does not represent an endorsement by the U.S. Department of Health and Human Services or guarantee the receipt of incentive payments.
