New Jersey’s Kennedy Health System Selects eClinicalWorks for DSRIP Program

eClinicalWorks, a leader in ambulatory healthcare IT solutions, today announced that New Jersey’s Kennedy Health System has selected eClinicalWorks Care Coordination Medical Record (CCMR) for population health management and the eClinicalWorks Electronic Health eXchange (eEHX) interoperability platform. These technologies will aid Kennedy Health System in addressing the objectives of the Delivery System Reform Incentive Payment (DSRIP) program, along with achieving measurable, incremental clinical outcome results demonstrating the initiative’s impact on improving healthcare delivery in the state.

The DSRIP Program provides hospitals with funding to improve quality of care and is a major component of New Jersey’s Comprehensive Medicaid Waiver. DSRIP is designed to result in better care for individuals, better health for the population, and lower costs by transitioning hospital funding to a model where payment is tied to achieving health improvement goals. Similar programs are being pursued in the state of New York.

“At Kennedy, we have made it our top priority to continually improve patient outcomes through evidence-based care processes,” said Dr. Carman Ciervo, Chief Physician Executive. “We selected eClinicalWorks because it provides us with the best overall solution to our meet our population health and DSRIP performance goals. eClinicalWorks has shown itself to be a strategic partner as we continue to pursue quality of care initiatives.”

eClinicalWorks CCMR will provide Kennedy Health System with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. The eEHX health information exchange will connect a variety of systems, creating a community record that will provide physicians with access to patients’ vital health information and allow physicians to exchange data more effectively.

Depending on the state, one component of the DSRIP Program encourages organizations to be recognized as a Patient-Centered Medical Home (PCMH). eClinicalWorks’ solutions are NCQA pre-validated and provide organizations with 16.25 auto-credit points towards their PCMH recognition. As a result of investments by the company in PCMH, 70 percent of Medical Homes using eClinicalWorks have achieved Level 3 recognition, the highest level possible.

“The DSRIP program offers hospitals a chance to improve patient outcomes and reduce the cost of care,” said Girish Navani, CEO and co-founder of eClinicalWorks. “Our population health platform, interoperability capabilities and strong track record with Patient-Centered Medical Homes will enable Kennedy Health System to benefit from the DSRIP program along with several other quality of care initiatives.”

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
