Greenway Health Solutions NCQA PCMH-Prevalidated to Foster Nationwide Care Coordination, Improved Outcomes
Greenway Healthcontinues to lead efforts to improve nationwide population health and care coordination by delivering solutions that support value-based programs such as the National Committee for Quality Assurance (NCQA) patient-centered medical home. PCMH is a primary care delivery model designed to improve health outcomes and reduce healthcare costs while enhancing patients’ and providers’ care experience.
Greenway’s SuccessEHSelectronic healthcare record (EHR) and practice management (PM) solution is now prevalidated by NCQA to receive 27 points in auto credit toward PCMH 2011 scoring — one of the highest information technology solution scores published to date.
All three Greenway® EHR/PM solutions have now achieved prevalidation status for NCQA PCMH, including PrimeSUITE® and Intergy™. Using any of these Greenway solutions, practices can streamline the reporting process associated with participating in this important value-driven initiative.
NCQA PCMH is a method to organize primary care practices into medical homes that emphasize care coordination and communication to transform primary care into what NCQA calls “what patients want it to be.” Medical homes can lead to both higher quality and satisfaction as well as lower costs, and NCQA states that its PCMH program is the most widely used way to transform these practices into medical homes.

The PCMH program and medical homes encourage healthcare consumers to more fully engage in managing their own health, while helping providers leverage the most effective tools to deliver care and coordinate with other organizations such as specialist practices, hospitals, labs and pharmacies to prevent errors and unnecessary duplication of care.
PCMH reporting can also facilitate a practice’s participation in other quality reporting incentive programs such as meaningful use and the Physician Quality Reporting System (PQRS), which have like-minded goals for preventive care, chronic disease management and patient compliance.
