Puerto Rico’s Metro-North ACO Selects eClinicalWorks
WESTBOROUGH, Mass.—February 5, 2014—eClinicalWorks, a leader in ambulatory healthcare IT solutions, today announced that Puerto Rico’s Metro-North ACO has selected eClinicalWorks Care Coordination Medical Record (CCMR) for advancing its physician-led Accountable Care Organization (ACO) objectives, including coordinating care across the continuum, evaluating population health and quality improvement activities.
“Our organization is focused on the patient experience and making sure every member is engaged in offering the best care possible,” said Dr. Ismael Toro of Metro-North ACO. “With 500 providers and growing, we need to exchange data between 12 different electronic health records and other health information systems. eClinicalWorks is a highly-comprehensive population health management tool for connecting systems, building a referral network, and conducting analytics and care planning. Using eClinicalWorks CCMR will allow us to be the first physician organization in our area to provide risk assessment and predictive analytics.”
The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. Metro-North ACO participates in the Medicare Shared Savings Program, which has been created by the Centers for Medicare & Medicaid Services (CMS) for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries and reduce unnecessary costs. eClinicalWorks CCMR will aid Metro-North ACO in meeting its quality measures and performance metrics.
“Metro-North ACO is bringing the benefits of value-based care delivery to Puerto Rico,” said Girish Navani, CEO and co-founder of eClinicalWorks. “We are pleased to be working with Metro-North ACO as its providers navigate through this care delivery model and take advantage of the benefits achieved through data sharing and point-of-care analytics. eClinicalWorks has made significant investments in its population health management tools, offering the most comprehensive, vendor-neutral solution in the market today to give providers all of the information and analysis required to manage patient populations and make even more informed actions at the point-of-care.”
eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
