eClinicalWorks, a leader in ambulatory healthcare IT solutions, today announced that New Jersey’s Kennedy Health System has selected eClinicalWorks Care Coordination Medical Record (CCMR) for population health management and the eClinicalWorks Electronic Health eXchange (eEHX) interoperability platform. These technologies will aid Kennedy Health System in addressing the objectives of the Delivery System Reform Incentive Payment (DSRIP) program, along with achieving measurable, incremental clinical outcome results demonstrating the initiative’s impact on improving healthcare delivery in the state.
The DSRIP Program provides hospitals with funding to improve quality of care and is a major component of New Jersey’s Comprehensive Medicaid Waiver. DSRIP is designed to result in better care for individuals, better health for the population, and lower costs by transitioning hospital funding to a model where payment is tied to achieving health improvement goals. Similar programs are being pursued in the state of New York.

eClinicalWorks CCMR will provide Kennedy Health System with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. The eEHX health information exchange will connect a variety of systems, creating a community record that will provide physicians with access to patients’ vital health information and allow physicians to exchange data more effectively.
Depending on the state, one component of the DSRIP Program encourages organizations to be recognized as a Patient-Centered Medical Home (PCMH). eClinicalWorks’ solutions are NCQA pre-validated and provide organizations with 16.25 auto-credit points towards their PCMH recognition. As a result of investments by the company in PCMH, 70 percent of Medical Homes using eClinicalWorks have achieved Level 3 recognition, the highest level possible.

Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
